Access to affordable medications is a top priority for Eli Lilly and Company. LillyMedicareAnswers is a patient assistance program that provides eligible Medicare recipients access to affordable medications outside of their Medicare Part D plan.
The LillyMedicareAnswers program provides assistance to eligible patients prescribed FORTEO® (teriparatide [rDNA origin] injection), ZYPREXA® (olanzapine), or HUMATROPE® (somatropin [rDNA origin] for injection).
Some of the key eligibility criteria for the program are:
- Enrollment in a Medicare Part D plan
- Not eligible for both Medicare and Medicaid (non-dual eligible people)
- Income at or below the program´s set percent of the Federal Poverty Level (FPL)
- Proof of low-income subsidy denial¹
- Signed certification that the patient will not claim or submit any cost incurred or associated with the
medications provided under the LillyMedicareAnswers program towards their true-out-of-pocket-cost (“TROOP”) calculation
or submit any claim for reimbursement for the medications²
Once patients have enrolled in the LillyMedicareAnswers program, they are eligible to participate in the program
for the rest of the calendar year, as long as they have a valid prescription for the medication. Patients must
re-enroll in the program each year.
Please click here for a copy of the enrollment application.
Please print the application, complete, and mail to: LillyMedicareAnswers, PO Box 66977, St. Louis, Missouri 63166-6977.

Patients will receive a letter notifying them if they have been enrolled in the program. The enrollment process and medication shipment
to the patient will require approximately 2 weeks from the time the complete application, documentation, and prescriptions are received.
If the prescription is for FORTEO or Humatrope, patients are required to include a prescription for the injection supplies. Medication

1. A copy of the denial letter from the Social Security Administration is required. Patients with income below 135% FPL must apply and be denied access to the Federal low-income subsidy. Patients with income between 135% and 150% of the Federal low-income subsidy are encouraged to apply for the subsidy but will be eligible for LillyMedicareAnswers regardless of approval or denial for the subsidy.
2. A signed application indicates that the patient understands and certifies this requirement.
